Tayside Palliative Care Conference 2019 have identified four umbrella "communities" in which we strive to deliver truly holistic palliative care. These are patient's own home, care homes, hospices and hospitals.
At the up and coming Tayside Palliative Care Conference 2019 we will explore the challenges faced by teams in delivering holistic care across these communities. We are inviting four plenary speakers, to discuss with us their insight how we can improve the experiences of patients as they experience care within, and transition between these communities.
